During that inspection, investigators cited Manorcare Health Services for the following violations of the federal Nursing Home Reform Act:
F 154 Failing to fully inform a resident of about care and treatment and any changes in care and treatment that may affect the resident’s well-being;
F 279 Failing to develop a care plan to address a resident’s needs;
F 282 Failing to provide services by qualified persons in accordance with each resident’s plan of care;
F 309 Failing to provide the necessary care and services to attain or maintain each resident’s highest practicable physical, mental and psychosocial well-being;
F 356 Failing to post daily nurse staffing information; and,
F 465 Failing to provide a safe, functional, sanitary and comfortable environment for residents, staff and public. “On 3/2/16 at 9:27 a.m., the sink in the dirty utility room on the 2nd floor east, the back splash was found to be rotted, stinky and coated with a black substance.”
