Bay Breeze Health and Rehabilitation Center is a 120-bed facility. This nursing home is located at 1026 Albee Farm Road, Venice FL 34285 and is owned and operated by Consulate Healthcare. Consulate is one of the largest for-profit nursing home chains in the United States. The controlling interest for this nursing home is a limited liability company known as CMC II, LLC. An annual survey inspection was conducted at Bay Breeze Health and Rehabilitation Center and concluded on January 11, 2018 by the Florida Agency for Healthcare Administration. At that time, state investigators observed issues which they believed were serious enough to constitute violations of state and federal law.

These concerns included and related to the:
Failure to follow physician orders;
Failure to have or follow an adequate infection prevention and control protocol;
Failing to ensure that one or more residents are free from the administration of unnecessary drugs;
Failing to ensure that services that are provided to residents meet professional standards of quality; and,
Failing to provide a safe, clean, comfortable and homelike environment for residents.
In addition to those deficiencies, investigators also cited this Venice nursing home for not having an adequate background screening process in place to ensure that those individuals with criminal backgrounds are not hired and employed to care for those elderly individuals who reside there. The law provides that nursing home corporations must not employ individuals who, based on their criminal past, may be more likely to abuse and neglect elderly nursing home residents whom they are supposed to protect.
