Crescent Health lawsuits were filed with the Sarasota Circuit Court in 2025 and, additionally, new complaints brought investigators with the Florida Agency for Health Care Administration into this Sarasota nursing home during January, 2025. Crescent Health and Rehabilitation Center is a 140-bed nursing home located at 5401 Sawyer Road, Sarasota, Florida 34233. Crescent Operations, LLC has held the license to operate this Sarasota nursing home since May, 2023.
Prior to the most recent January, 2025 investigation, Florida officials were previously in the nursing home to conduct an inspection that concluded on September 12, 2024 with the citation of (18) deficiencies. A deficiency represents the failure to comply with the requirements for nursing homes under state or federal law.
Of the two most recent complaints investigated by Agency representatives, one was substantiated. Two deficiencies were cited against Crescent Health and Rehabilitation Center under both state and federal law at the conclusion of the investigation. Those deficiencies were in relation to all nursing homes’ obligation to maintain a safe, clean, comfortable and homelike environment for residents.
As of December, 2025, Crescent Health and Rehabilitation Center receives the lowest possible rating from (CMS) Medicare; only one star out of five possible stars. In 2024, CMS fined the nursing home over $26,000.00 based on its inspection results.

Four Open Crescent Health Lawsuits Alleging Violations of Resident Rights and Negligence are Pending in Sarasota Courts as of December, 2025
The filing of four separate and distinct Crescent Health lawsuits with the Sarasota County Clerk of Court naming the nursing home’s corporate operator as a defendant occurred within the past fifteen months. Each lawsuit alleges that Crescent Operations, LLC held the license to operate Crescent Health and Rehabilitation Center and that this company acted negligently and violated the resident rights of each resident.
In the first lawsuit brought against Crescent Health, it is alleged that the nursing home failed to provide proper care and services to prevent the development and worsening of wounds and that they failed to timely transfer him to the hospital for treatment.
In the second lawsuit, it is alleged that the nursing home and its staff failed to provide necessary care, including incontinence care, to a female resident to prevent the worsening and infection of a pressure wound. The complaint seeks damages for her pain and suffering, mental anguish, and medical expenses.
In the third lawsuit filed against Crescent Health’s corporate owner, it is alleged that the nursing home failed to provide the proper care and interventions to prevent a female resident from falling and suffering injury. The complaint seeks damages for negligence and wrongful death.
In the fourth lawsuit, the nursing home and its staff are alleged to have failed to properly care for a female resident’s traumatic injuries during an admission for rehabilitation, resulting in the resident suffering infection and a deterioration in her health.
Each of these Crescent Health and Rehabilitation Center lawsuits remain open and pending at this time in Sarasota County, Florida.
