An 81 Oaks Senior Living complaint investigation, along with a relicensure inspection conducted at this Sarasota assisted living facility, resulted in the citation of deficiencies by the State of Florida. The license holder for this ALF, located at 7850 Hawkins Road, Sarasota, Florida 34241, is a for-profit corporation known as 81 Oaks LLC. It has a total capacity of 125 beds and offers limited nursing services.
On September 30, 2025, investigators with the Florida Agency for Health Care Administration conducted a complaint investigation along with a standard relicensure inspection at the facility. Multiple deficiencies were observed, documented and cited by Florida officials at that time.
81 Oaks Senior Living Complaint Investigation and Relicensure Inspection Reveals Thirteen (13) Violations under Florida Law
Florida investigators cited the license holder of the assisted living facility for failing to comply with thirteen (13) separate requirements under Florida law. One of the violations occurred when staff at the facility were observed assisting residents with the self-administration of medications. Based on firsthand observation, a review of records and interviews of employees, Florida officials determined that “the facility failed to ensure staff assisting with the self administration of medications do so appropriately and perform hygiene” when assisting all five of the residents that this impacted. The facility policy required staff to wash their hands or use an appropriate hand sanitizer prior to assisting with medications. This apparently was not being done. In addition to multiple violations related to staff training, the ALF was also cited for failing to comply with food service dietary standards. In this instance, the physicians for two residents had ordered that they receive a therapuetic diet based on their health conditions. These residents both required that food be mechanically altered before having it served to them. It is extremely important that these physician diet orders be strictly adhered to because the failure to do so can result in choking, asphyxiation and death. An inspector observed that one resident eating lunch who required a mechanical soft diet was served a meal that contained “[f]ull sized, fresh cooked broccoli florets.” Another resident who was ordered to receive a mechanically ground diet was seen eating a meal that was “cut into large pieces and and not ground as per physician diet orders.”
