
Discovery Commons South Biscayne, a North Port, Florida assisted living facility, was cited with multiple training-related deficiencies following an inspection by officials with the Agency for Health Care Administration in late October. This for-profit 147-bed facility is located at 6235 Hoffman Street, North Port, Florida 34287.
The inspection occurred as a result of this ALF’s biennial license renewal. Multiple failures to comply with Florida law were identified by Florida officials at that time.
Discovery Commons South Biscayne Determined to Have Violated Florida Law for Failing to Provide Staff Training

When inspectors with the State of Florida conducted their survey, they reviewed records, made observations and interviewed employees at the ALF. Given time limitations, these investigators will normally review a sample files rather than look at every single record within the building. In this instance, three employment files were pulled for review. Of those three employees, it was determined that two of them had failed to receive the mandated training within 30 days of being hired:

As a Florida assisted living facility that provides extended congregate care, Discovery Commons was required to have the administrator or ECC supervisor provide an additional two hours of in-service training within six months of hire. None of the sampled employees received that training according to state investigators. A second Class III deficiency was cited for this violation. Additional violations were found in relation to the failure to provide required training in the areas of Alzheimers Disease/Dementia and ‘do not resuscitate’ orders.
Has this North Port, Florida assisted living facility been the subject of legal actions? The answer is yes.
