Discovery Village at Sarasota Bay is a for-profit 150-bed assisted living facility located at 1414 West 69th Avenue, Bradenton, Florida 34207. The company that is the controlling interest behind the license holder is Discovery Senior Living. The current licensee of this ALF took over operations in 2021. Since that time, a review of public records reflects that multiple complaints have been made about this facility.
During 2023, Florida investigators with the Agency for Health Care Administration inspected the ALF and documented violations and failures to comply with Florida law. Deficiencies were issued. Since the change in ownership occurred in 2021, the ALF license holder has been cited with a total of six deficiencies by the State of Florida. The deficiencies were generally related to staff training and resident records. As indicated, the facility received numerous complaints during 2023 that were investigated by officials with AHCA.
When complaints are substantiated and deficiencies are issued the State of Florida has a range of options available depending on the scope and severity of the violations.
One option available to AHCA is to bring a legal action and seek the imposition of a fine against an assisted living facility and its operator.
In this instance, the Florida Agency for Health Care Administration took action and imposed two administrative fines against the license holder / owner of this ALF during 2023.
The most recent fine was imposed on December 13, 2023 by the Agency.
