Elison Assisted Living of Bella Vita complaints were received and investigated at this Venice, Florida ALF by the Agency for Health Care Administration. Elison Assisted Living of Bella Vita is a private 130-bed for-profit ALF located at 1420 East Venice Avenue, Venice, Florida 34292.

Three separate complaints were investigated in November by Florida officials. The results were published and detailed in a fifteen page report.

State inspectors made observations, interviewed staff and reviewed records in conducting their investigation at this Venice assisted living facility. The first violation that was discovered involved residents who were considered to be at risk for elopement. Elopement occurs when a resident, without detection, wanders away from an asissted living facility or nursing home. Of the four residents who were deemed at risk, the facility failed to meet requirements under Florida law for all four of them by failing to ensure that they had proper identification on their persons.

Florida assisted living facilities are also required by law to maintain their premises in a safe, clean and working order. Florida officials issued a deficiency for the ALF’s failure to meet this requirement. Inspectors noted in their report “several large stains in the carpeting observed with a distinct smell of [redacted].” On the second floor, an investigator noted the existence of “several stains observed in the carpeting along with a foul odor coming from the carpeting.” And carpeting “in the dining room was observed to be heavily soiled and stained.” When asked, several residents also complained about these issues.

An additional deficiency was issued based on the asissted living facility’s failure to comply with the requirements for resident contracts.

These prior violations were outlined in a 26-page report and statement of deficiencies published by the Florida Agency for Health Care Administration. All ALFs in Florida are required by law to make their inspection results available for viewing within the facility. These assisted living facilities are inspected whenever the State of Florida receives a complaint and also periodically close to the two-year time period when an ALF’s license is up for renewal.

While conducting their unannounced investigation at this Venice assisted living facility, Florida officials observed “unlicensed staff assisting with medication self-administration,” and failing to “orally advise residents of the doses” as well as failing to “orally advise residents of medication names.” Inspectors also noted that unlicensed staff members failed to sanitize their hands before doing so. During the inspection, they also witnessed that medication was being stored in a manner that was in violation of Florida law, specifically Section 59A of the Florida Administrative Code that applies to the operation of assisted living facilities. Based on these observations, the facility received citations of two Class III deficiencies related to medication assistance and medication storage and disposal.

Additionally, during this inspection, Florida officials reviewed records and files and determined that this Venice ALF failed to comply with a number of standards and requirements under the law relating to staffing and training at the facility. Five (5) additional Class III deficiencies were cited against the facility in that regard.
