
The Meridian at Punta Gorda Isles complaints brought investigators with the Florida Agency for Health Care Administration to this assisted living facility to conduct an inspection. This for-profit facility is located at 2001 Aqui Esta Drive, Punta Gorda, Florida 33950 and is licensed to provide 160 beds.
A thirteen page report prepared by inspectors with the Agency reveals that four separate complaints were investigated at this Punta Gorda assisted living facility.
The Meridian at Punta Gorda Isles Cited for Failing to Update Resident Health Assessment and Properly Maintain Medication Records
One of the issues investigators looked at was the fitness or appropriateness of residents to remain in an assisted living environment following their admission. As individuals age and time passes, health conditions can deteriorate, requiring a resident to be discharged from a facility if they can no longer be safely cared for in an ALF. This process requires that the Form 1823 resident health assessment be periodically updated; this is especially necessary after a documented incident or medical issue becomes apparent to staff.
At the time of the investigation, Florida officials reviewed records, conducted interviews and made observations. They determined that the facility failed to update one resident’s health assessment after that resident had eloped from the facility. The “Wellness Director acknowledged Resident #9’s 1823 had not been updated to reflect the change in condition for elopement status.”
In another instance, Florida investigators determined that The Meridian at Punta Gorda Isles failed to comply with the law concerning medication records. The report states that the law was “not met as evidenced by: Based on observation, record review, and interview, the facility failed to chart missed dosages or medication errors for” a resident. An additional Class III deficiency was cited based on the facility’s failure to maintain proof that required training had been provided to staff.
