
The Fountains of Hope complaints and scheduled routine inspections brought investigators with the Florida Agency for Health Care Administration to this Sarasota ALF twice in 2025. This for-profit 116-bed assisted living facility is located at 2250 Jesus Way, Sarasota, Florida 34240.
Since 2021, this facility has received a total of eight (8) deficiencies cited for violations of Florida law. The most recent deficiencies are detailed below.
The Fountains of Hope Found in Violation of Florida Law During Recent Inspections

Most recently, a complaint resulted in the ALF receiving a deficiency for failing to comply with the risk management / quality assurance provision under Florida law. Florida officials visiting the facility reviewed records, conducted interviews and made observations at that time. Whenever a resident of an ALF suffers an adverse incident that requires the resident be transferred out to receive a higher level of care, a report of the incident must be reported electronically to the Agency. Investigators reviewed a total of thirteen incident reports and found that two of the incidents were never reported to the State of Florida as required.
An earlier investigation at the ALF found numerous issues for which deficiencies were cited. In Florida ALFs, unlicensed personnel are permitted to assist residents with the self-administration of medications. They must, however, do so in accordance with the law and facility policies and procedures. State investigators observed a staff member failing to inform a resident of the name and dosage of a medication while providing that assistance, resulting in a deficiency. At the time of this inspection, the facility also received deficiencies relating to staffing standards and training. A total of four (4) Class III deficiencies were cited.

Fountains of Hope Assisted Living in Sarasota Sued in Breach of Contract Lawsuit in January, 2026 By Creditor

On January 27, 2026, a lawsuit was filed against this Sarasota assisted living facility alleging that the company that holds the license to operate the ALF breached a contract to pay for leased equipment. The equipment identified in the lease is the Ensure360 system made by Sentrics. According to the manufacturer’s website, the “Ensure360 is a life safety and emergency call (eCall) technology platform designed for senior living communities, developed by Sentrics. It is part of the Sentrics360 suite, which is designed to monitor the physical, medical, and behavioral needs of residents in real time.”
This lawsuit follows administrative lawsuits brought by the State of Florida seeking the imposition of fines as penalties for failing to comply with Florida law applicable to assisted living facilities.
