North Port Rehabilitation and Nursing Center is a for-profit 120 bed nursing home located at 6940 Outreach Way, North Port, FL 34287. All Florida nursing homes are periodically inspected by state officials with the Florida Agency for Health Care Administration. These routine inspections are referred to as “surveys.” When an inspection team goes into a nursing home, they do so unannounced. These inspections occur at least once every fifteen months and whenever a complaint is reported to the Agency. Once an inspection is complete, the Agency’s findings are documented in a report form known as the Form CMS-2567 Statement of Deficiencies and Plan of Correction. A nursing home that receives deficiencies must either timely appeal the findings or file a plan of correction that fully addresses the documented failures to comply with state and / or federal law. Fines may also be imposed.
