Windsor Reflections at Lakewood Ranch is a for-profit 62-bed assisted living facility located at 8230 Natures Way, Bradenton, Florida 34202. Over the past couple of years, this ALF has been the subject of a number of complaints made to the Florida Agency for Health Care Administration. Once a complaint is received, investigators with AHCA send in a team of investigators on an unannounced basis to compile and review evidence. Interviews are conducted with facility employees, administration and residents. Since 2020, this assisted living facility has been cited with seven Class III deficiencies by the State of Florida.

Windsor Reflections at Lakewood Ranch complaint investigations conducted at the facility have largely been related to issues involving medications. One of the most important aspects of assisted living facility care is the handling and dispensing of medications to residents and keeping an accurate record of doing so.
Specifically, this assisted living facility has been cited for violating Florida law relating to medication records, the background screening of employees, the assistance with the self-administration of medications and failures regarding medication storage and disposal.
The most recent deficiency was cited during 2023. Florida officials spoke with facility employees and reviewed the medication administration records for residents. Investigators determined that the ALF violated Florida law concerning the maintenance of daily medication observation record for residents. The Agency cited the facility and stated that it “failed to immediately update the daily observation record for residents receiving assistance with self-administration for two (Resident #1 and Resident #2) of the two sampled residents.” According, the ALF was cited with a Class III deficiency by the State of Florida.
