
Two Creekside Health lawsuits were filed in 2025 that name the foreign limited liability company, Creekside Rehab Opco LLC, as a defendant. The Sarasota nursing home fictitiously known as Creekside Health and Rehabilitation Center is a 178-bed facility located at 5511 Swift Road, Sarasota, Florida 34231.
Creekside Health and Rehabilitation Center was inspected by Florida officials as part of its recertification process during 2025. The nursing home’s operator was most recently cited for failing to comply with both state and federal requirements under the law at that time. The deficiencies, among others, cited related to the failure to maintain a safe, clean, comfortable and homelike environment for residents.

This Sarasota nursing home was cited with a total of eighteen (18) deficiencies on the year as of December, 2025. These violations and failures relate to infection prevention and control, quality of care, and the failure to follow physician orders.

Creekside Health Lawsuit Seeks Damages for Negligence and Wrongful Death

The first Creekside Health lawsuit was filed in 2025 in the Twelfth Judicial Circuit Court in and for Sarasota County. The complaint alleges that Creekside Health and Rehabilitation Center was negligent in caring for a gentleman who was admitted to the facility for care and supervision. According to the complaint, the resident sought services from the nursing home “following a traumatic motor vehicle collision which resulted in both a brain bleed and a fractured neck” and that he relied on staff for assistance.
The lawsuit further alleges that the nursing home “failed to provide any monitoring or fall prevention mitigation strategies” to protect the resident, and as a result, he “attempted to get out of his bed on his own and fell causing significant and severe injuries and death.”
The second lawsuit alleges that the nursing home was negligent in caring for a gentleman, and thereby allowed him to suffer “the development and deterioration of pressure wounds.” Pressure wounds, or bedsores as they are commonly known, typically result when an incontinent resident is permitted to lie in the own waste. The heat and moisture, along with unrelieved pressure to an area such as the backside, can quickly lead to skin breakdown.
Both lawsuits remain open and pending at this time.
