Prior to this most recent inspection, a complaint against the facility led to an investigation into the facility’s risk management and adverse incident reporting practices. The issue involved the failure of the ALF to submit the proper documentation to AHCA following the investigation of an adverse incident. Florida officials interviewed staff members. The Administrator, who also happened to be a Licensed Practical Nurse, admitted to the AHCA investigator: “I guess we didn’t do it.” The State of Florida issued a Class III deficiency against the ALF.
