Creekside Health and Rehabilitation Center is a 178-bed nursing home located at 5511 Swift Road, Sarasota, Florida 34231. Complaints made against this Sarasota nursing home recently brought a State of Florida inspection team into the facility to conduct an unannounced investigation.

All nursing homes in Florida are required to maintain a safe, clean and homelike environment for their residents. In fact, this is one of the various “resident rights” guaranteed to nursing home occupants under the law. Investigators with the Florida Agency for Health Care Administration documented in their report that on September 18, 2024, they observed conditions within the memory care unit at the nursing home that they concluded did not meet this requirement. Inspectors noted the existence of “[w]allpaper peeling away from the wall above the floor, along the wallpaper seams and along the ceiling… missing cove moldings, [s]heetrocks were cracked with holes noted in the walls.” Other issues were noted in the memory care shower room as well. Florida officials met with the nursing home administrator to show him what they had found. According to investigators, the administrator “observed and verified the broken blinds in the shower room, the broken lights in the bathroom and shower stall and the floor tile covered with orange and brown film.” He also verified the existence of the other issues.
The Agency cited the nursing home for failing to comply with those legal requirements under both state and federal law that guarantee a safe, clean, comfortable and homelike environment for residents.
