
Complaints at Charlotte Bay Rehab and Care Center were investigated by inspectors with the Florida Agency for Health Care Administration in 2024. This facility is a for-profit 164-bed nursing home that is located at 4033 Beaver Lane, Port Charlotte, Florida 33952.
Florida investigators conducted an unannounced inspection at the nursing home at which time they reviewed clinical records, interviewed staff members and spoke to residents and their family members. The primary basis for the complaint appears to be that the nursing home was failing to provide the “necessary care and services to maintain personal hygiene” for residents.

The complaint investigation resulted in the discovery of violations under both state and federal law. Of note, this Port Charlotte nursing home was found to have not met requirements related to the maintenance of the nutrition and hydration status of residents, failing to follow physician orders, failing to adhere to the right to receive adequate and appropriate healthcare, and the failure to ensure a safe, clean and homelike environment for residents.
