The Meadows Center complaints received by the Florida Agency for Health Care Administration were investigated contemporaneously with an unannounced licensure recertification inspection at this Sarasota skilled nursing facility during October, 2024. Quite a few citations of deficiencies were issued at that time. The Meadows Center for Nursing and Healing is a for-profit 120-bed nursing home located at 5157 Park Club Drive, Sarasota, Florida 34235.
Last year, this Sarasota nursing home was the subject of an administrative complaint brought by the Agency. A Final Order entered in the case required the operator of The Meadows Center for Nursing and Healing to pay the Agency $50,500.00 in the form of $38,500.00 as a fine and survey costs in the amount of $12,000.00.
The Meadows Center for Nursing and Healing Remains on the Florida “Nursing Home Watch List”
As specified by AHCA, “[t]he Watch List identifies nursing homes that are operating under bankruptcy protection or met the criteria for a conditional status during the past 30 months. A conditional status indicates that a facility did not meet, or correct upon follow-up, minimum standards at the time of an inspection. Immediate action is taken if a facility poses a threat to resident health or safety.” In this instance, the nursing home’s license was downgraded to conditional status for a total of 89 days during 2023.
Multiple Violations Cited During 2024 Inspections
Most recently in 2024, the facility was cited with a total of sixteen deficiencies, many of which related directly to nursing home resident care.
The Meadows Center for Nursing and Healing was cited in relation to the failure to ensure the accuracy of assessments, failing to ensure the timely completion and revision of care plans, failing to provide care and services to dependent residents, failing to maintain an effective pest control program, and failing to ensure a safe, functional and sanitary environment for residents of the facility. Three Lawsuits Filed Against The Meadows Center Operating and Management Companies in 2024
Of the three The Meadows Center lawsuits filed by former residents through their representatives and the estate of a deceased resident, two remain open and pending at this time. The third case was settled at mediation. Each of these lawsuits alleged that The Meadows Center for Nursing and Healing was negligent in providing care and services to each respective resident, and that those acts and omissions resulted in injury and harm. With respect to the resident who died, a wrongful death lawsuit has been filed against the nursing home and its corporate operators and managers. That lawsuit remains open and pending at this time.
